In this role, you will provide direct patient care for 6-10 seniors daily. These seniors want an alternative to long term care placement. and to live with dignity in their communities. PACE participants can remain in the secure surroundings of their own homes and communities while providing family members and care givers with much needed professional guidance, physical relief and emotional support. PACE is a place to go to meet new friends, participate in fun activities and receive your medical, social and emotional care. While most care is delivered in the adult day health center during the day, care is coordinated 24 hours a day. PACE's day-to-day monitoring of participants results in fewer hospitalizations, less reliance on prescription medications, better quality of life and better overall health.
