opportunity for a strategic, creative, collaborative, and
mission-oriented professional to serve as Director of Care
Management at UVA Health.The Director of Care Management (Director)
leads the planning, management, and evaluation of the Care
Management activities, functions, and workforce for UVA Health.
They are responsible for staffing within Care Management and
advocating for resources where needed. Working across all service
lines and medical departments within UVA Health, the Director
oversees a portfolio of teams, including Case Management, Social
Work, and Chaplaincy Services. Their responsibilities include
modernizing and implementing best practices; standardized
processes, roles, and systems across all four functional areas; and
integrating services into a cohesive, patient-centric team.The
Director reports to the Chief Nursing Officer (CNO) of UVA Health.
In conjunction with the CNO and the Chief Medical Officer (CMO),
the Director establishes metrics for care management to meet
regulatory requirements and goals of UVA Health and for achieving
targets to ensure efficient throughput, effective complex care
management, and clinical and operational excellence.The Director
promotes effective patient progression across the continuum of care
in collaboration with UVA Health senior leadership, medical staff,
advanced practice providers, nursing staff, patient financial
services (revenue cycle), utilization management, and other
departments. In particular, the Director will partner
collaboratively with the Director for Patient Flow Operations and
Strategy on patient transition performance improvement
opportunities. The Director ensures care management initiatives and
programs are coordinated with the Patient Placement Center,
Transitional Care Hospital, and UVA Continuum Home Health.
Additionally, they play an integral role in building a robust care
management network community across the State of Virginia to
partner with UVA Health.This is a full-time, on-site leadership
position and COVID-19 vaccination and boosters are required for UVA
Health employees.QualificationsThe Director must be strategic,
analytic, creative, inclusive, and collaborative. They will display
discretion and good judgment and will demonstrate the ability to
get things done while working effectively across a complex
institution in a highly matrixed environment. They must be a person
of integrity whose beliefs and actions align with and advance UVA
Health's mission, values, and stated policy objectives, including

be an experienced leader able to listen to various perspectives,
synthesize information, manage budgets, make decisions, communicate

areas. In addition, the Director must be aware of best practices
and emerging trends within care management and have experience
implementing them across a complex health system environment. An
appreciation for the tripartite mission of excellence in clinical
care, research, and education is important, as is an understanding
of the implications of being a teaching organization when
considering process flow and efficiencies.The Director must be a
big-picture thinker who understands how to translate vision to
action and is goal-driven, systems-oriented, data-focused, and
willing to embrace and lead change while maintaining focus on UVA
Health's mission and the people at the heart of it all.
Qualifications for the role include: 5 years of progressively
responsible leadership experience within a healthcare context,
Bachelor's degree in nursing from an accredited institution,
Master's degree completed within 4 years of hire, Applicable
specialty and/or administration certification within 3 years of
hire License to Practice as a Registered Nurse in the Commonwealth
of Virginia, and American Heart Association (AHA) Health Care
Provider BLS certification. In addition to these requirements, the
strongest candidates will also have more than 5 years of leadership
experience in healthcare; a master's degree or higher; and
applicable specialty and/or administration certification.To
