*Overview*We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care and ensuring optimal outcomes. If you are passionate about providing quality healthcare services and making a positive impact on patients' lives, we want to hear from you.*Duties*- Conduct comprehensive assessments of patients' health status- Develop and implement individualized care plans- Coordinate with healthcare providers to deliver integrated care- Monitor patient progress and adjust care plans as needed- Provide patient education on treatment plans and self-care strategies- Collaborate with interdisciplinary teams to optimize patient outcomes- Utilize medical records and medical terminology effectively- Engage in discharge planning and follow-up care arrangements*Minimum 2 Years' experience as a case manager RN*Genie Healthcare is currently seeking experienced Travel Case Manager RNs to fill an opening located in Charlottesville, Virginia!$2,424 per week gross for 40 hrs/week5x8 hour days, 2-3 weekends during contract*Requirements:* 2+ years of case management experience-Must have pediatric case management experience VA or compact RN license-BLS (American Heart Association)-Must have acute care hospital inpatient CM experience8 hour day shift - 13 week contractJob Types: Full-time, Contract, Travel nursingPay: $2,424.00 per weekBenefits:* 401(k)* 401(k) matching* Dental insurance* Health insurance* Life insurance* Paid time off* Vision insuranceSchedule:* 8 hour shift* Day shiftExperience:* Nursing: 1 year (Preferred)* Case management: 1 year (Preferred)License/Certification:* RN (Preferred)Ability to Commute:* Charlottesville, VA (Required)Ability to Relocate:* Charlottesville, VA: Relocate before starting work (Required)Willingness to travel:* 25% (Preferred)Work Location: In personby Jobble
