Overview:
The Nurse Manager will provide leadership, coordination, and professional services to seniors through an interdisciplinary approach. The Nurse Manager will manage and lead our team of staff nurses and CAREGivers in order to provide the highest level of care to clients in-home and facility settings. Position requires at least 2 years supervisory experience or equivalent as well as clinical experience.
Primary Responsibilities:
Reflect the Company mission and core values
Implement the increase scope of care as allowed by franchisor
Review and update local policies and procedures annually or as needed
Ensure franchise is following VDH guidelines by understanding and implementing all requirements for licensure
Teach Personal Care curriculum and assess CAREGiver competencies
Perform assessment of Clients requiring personal care services or any Client requiring an assessment
Develop corresponding Plan of Care based on collaboration with Client and/or Client's family
Coordinate Client Assessments with staff RNs and/or Client Care staff
Provide quality assurance Client/family calls or visits
Communicate Client care needs/issues/changes with scheduled CAREGivers and office staff
Perform/coordinate Client Supervisory Assessments every 60-90 days per VDH regulations
Oversight of and support to RNs/LPNs to ensure consistency and adherence to Asilinge Caregivers policies, procedures, and protocols along with compliance with VDH and corporate standards
Document all information regarding Clients and CAREGivers in Company system utilizing appropriate tags and documentation processes
Audit Client/CAREGiver files as appropriate
Consult with staffing coordinator/s to ensure CAREGivers are assigned at the appropriate level of care
Establish and maintain strong relationships with neighboring Home Instead franchise teams
Lead the Quality Improvement Committee and participate in the Leadership Team
Review plans of care to ensure consistency and adherence to policies and procedures as well as Client needs share suggestions/changes with appropriate RNs and Client Care staff
Conduct RN performance reviews with input from Director of Client Care and franchise owner
Review of daily Dashboard tasks, addressing questions/concerns in a timely manner
Monitor the return of daily care notes to meet the VDH regulations
Assist in developing and enhancing HISC CAREGiver Training Program
Oversee continuing education requirements for Personal Care CAREGivers
Discharge clients from personal care service according to company policy and procedure
Demonstrate open and effective communication with owners, colleagues, and CAREGivers
Attend weekly staff meeting
Share on-call responsibilities with other Nursing staff members
Other duties as assigned
Secondary Responsibilities:
Conduct new client consultations, develop an initial plan of care following the consultative process, document appropriately, collaborate with Client Care team members to initiate care
Education/Experience Requirements:
Registered Nursing Degree from an accredited school of nursing
Current License by the Virginia Board of Nursing
Two years of supervisory experience
Must possess a valid driver's license and reliable, insured automobile
Clinical experience
Critical Numbers:
With RN/LPN ensure sufficient Personal Care Classes provided/completed
Ensure CAREGiver Training to deliver high-quality care resulting in high client satisfaction
Supervisory Responsibilities:
Supervision of Personal Care CAREGivers
Supervision of Client Care RNs
Knowledge, Skills, and Abilities:
Possess understanding of and uphold policies/procedures established by Asilinge LLC.
Commitment to continuous quality improvement
Possess a genuine passion for seniors
Be trustworthy and highly self-motivated
Demonstrate excellent oral and written communication skills and ability to listen effectively
Work independently, maintain confidentiality of information and meet deadlines
Demonstrate effective interpersonal skills essential as well as sound judgment and good decision-making skills

Organize and prioritize daily, monthly and yearly work
Establish good working relationships with colleagues, franchise owners, clients, and community
Able to sit at a desk and listen effectively for long periods of time on the telephone

Able to operate office equipment
Exhibit patience and congeniality
Able to input all data into HISC proprietary software
Knowledge of the senior-care industry
Able to perform duties in a professional office setting
