Job Description:
Exciting opportunity to join a team of highly motivated and
energized professionals. Must be able to function at a high level
with little oversight on a Monday - Friday day shift schedule,
coordinate and provide nursing care, training, education and home
support for patients. Must work collaboratively and develop
relationships with team members, patients and skilled nursing
facility representatives; collect and record (EMR) relevant patient
health data and develop patient care plans.BS or AA/diploma in
Nursing with at least 6 months of RN experience with 3 months in
hemodialysis needed. Need reliable daily transportation, valid RN
license, BLS certification, able to travel to local area sites and
