Job Description:
$5,000.00 relocation bonus, and $10,000 sign on bonus with a
2-year commitment for qualified candidates.
The UVA Children's Hospital Division of Pediatric
Hematology/Oncology seeks an RN Care Coordinator. The successful
candidate will join a dedicated and caring multidisciplinary team
consisting of MDs, NPs, a Licensed Clinical Social Work, a
Psychologist, Clinical Research Coordinators, an Education
Consultant and other professional staff to enable the best possible
service to these complex patients and their families. The role
involves management and coordination across the entire care
continuum in partnership with our providers and support staff. This
will include insuring proper scheduling of outpatient visits,
hospital admissions, procedures, imaging studies and other elements
of patient care, as well as securing prior authorizations for
treatment and coordinating care with home health providers.
Participation in pre-clinic care coordination conferences will be
required daily to insure a proactive approach to patient care
needs. Bi-directional communication with patients and families,
in-person, by telephone and via the electronic medical record is a
critical component of this position.
A dynamic professional clinician focused on population management
with demonstrated capability to provide relationship-based care
within a collaborative multidisciplinary team. This registered
nurse utilizes advanced critical thinking to develop, implement,
and evaluate an individualized plan of care that educates, empowers
and optimizes patient outcomes throughout all transitions within
the continuum of care.
This job description integrates the ANA 2015 Nursing: Scope and
Standards of Practice, Third Edition and the ANA Code of Ethics for
Nurses with Interpretive Statements (ANA, 2015), and the UVA
Nursing Professional Practice Model.

Nursing is the protection, promotion, and optimization of health
and abilities, prevention of illness and injury, facilitation of
healing, alleviation of suffering through the diagnosis and
treatment of human response, and advocacy in the care of
individuals, families, groups, communities and populations.
Education: Graduate of accredited nursing program, Bachelor of
Science in Nursing required within 5 years of hire.
Experience: 1 year of relevant experience.
Licensure: Licensed to Practice as a Registered Nurse in the
Commonwealth of Virginia. American Heart Association (AHA) Health
Care Provider BLS certification required.
PHYSICAL DEMANDS
Job requires standing for prolonged periods, frequently traveling,
bending/stooping. Proficient communicative, auditory and visual
skills; Attention to detail and ability to write legibly; Ability
to lift/push/pull 20 - 50lbs. May be exposed to chemicals,
blood/body fluids and infectious disease.
