Remote - New York, United States Type: Full Time Job Summary Our
client, a leading healthcare organization, is seeking a Registered
Nurse – Care Manager to support members through comprehensive care
coordination and ongoing clinical management. This role focuses on
improving health outcomes through proactive engagement,
collaboration, and personalized care planning. What You’ll Do
Conduct comprehensive member assessments within required timelines
to identify care management needs Develop and execute
individualized care coordination plans in partnership with members,
caregivers, physicians, and support networks Perform telephonic,
in-person, and home-based visits as needed Monitor and evaluate
care plans, documenting interventions, progress toward goals, and
recommending adjustments Manage an active caseload with consistent
outreach and follow-up Coordinate services across medical,
behavioral health, long-term services and supports, and
community-based resources Facilitate interdisciplinary care team
collaboration and meetings Use motivational interviewing techniques
to educate, engage, and support members in behavior change Identify
barriers to care and provide hands-on coordination to address
member challenges Support peers through consultation, guidance, and
shared best practices when needed Manage complex cases, including
medication-related needs and reconciliation when applicable Travel
locally (approximately 25–40%) based on program and regional
requirements What You’ll Bring Minimum of 2 years of experience in
healthcare, ideally in care management or clinical settings
(medical and/or behavioral health) Active, unrestricted Registered
Nurse (RN) license in the state of practice Valid driver’s license,
reliable transportation, and required auto insurance for
job-related travel Working knowledge of electronic medical records
(EMR) and healthcare privacy regulations Familiarity with
community-based resources and support services Strong
organizational skills with a proactive, detail-oriented approach
Ability to work independently while adapting to diverse populations
and care environments Excellent communication skills and the
ability to remain calm under pressure Proven time-management,
prioritization, and critical-thinking capabilities Comfort using
Microsoft Office and navigating online systems and databases
Preferred Qualifications Certified Case Manager (CCM)
