week (Monday through Friday, 8:30 a.m. – 5:00 p.m.) Location :
Lynchburg VA, 24501 Compensation dependent on experience Position
Summary The Case Manager serves as a support coordinator, ensuring
that services to individuals with developmental disabilities and
intellectual disabilities are planned, individualized, and
person-centered. The Case Manager provides services that may
include assistance to individuals and their family members in
accessing needed services that are responsive to the individual’s
needs. Required Education and Experience Bachelor’s degree from an
accredited college or university in an applicable human services
field A minimum of one (1) year of directly related work experience
providing case management to individuals with developmental
disabilities. May consider an equivalent combination of education
and experience that demonstrates the required knowledge, skills,
and abilities. Related Knowledge, Skills and Abilities Knowledge of
services, systems, and programs available in the community
including primary health care, support services, eligibility
criteria and intake processes, generic community resources, and
mental health, developmental disability, and substance abuse
treatment programs. Knowledge of the nature of developmental and
intellectual disabilities, serious mental illness, and substance
use disorders, and/or co-occurring disorders, depending on the
population served, including clinical and developmental issues.
Knowledge of different types of assessments, including functional
assessments (e.g., VIDES), and their uses in service planning.
Knowledge of treatment modalities and intervention techniques such
as behavior management, independent living skills training,
supportive counseling, family education, crisis intervention,
discharge planning, and service coordination. Knowledge of types of
developmental, mental health, and substance use services available
in the locality. Knowledge of the service planning process and
major components of a service plan. Knowledge of the use of
medications in the care or treatment of the population served.
Knowledge of all applicable federal and state laws, state
regulations, and local ordinances. Knowledge of identifying and
documenting an individual's needs for resources, services and other
supports. Knowledge of using information from assessments,
evaluations, observation, and interviews to develop individual
service plans. Knowledge of identifying services and resources
within the community and established service system to meet the
individual's needs; and documenting how resources, services, and
natural supports, such as family, can be utilized to achieve an
individual's personal habilitative/rehabilitative and life goals.
Knowledge of coordinating the provision of services by public and
private providers. Ability to work as a team member, maintaining
effective inter- and intra-agency relationships. Ability to work
independently, performing position duties under general
supervision. Ability to engage and sustain ongoing relationships
with individuals receiving services. Ability to complete DBHDS core
competency-based curriculum within 30 days of hire. Ability to
participate in and successfully pass behavioral intervention
training; includes both verbal de-escalation skills and physical
skills techniques to be utilized with consumers as needed. Willing
and available to work modified schedules (weekends, holidays, etc.)
as required. Ability to drive and must possess a valid Virginia
driver’s license. May be called upon to travel to any Horizon
worksite, transport clients, provide services to clients at other
locations. Regular attendance at training and Horizon meetings may
require travel to a variety of Horizon sites. The Best Benefits for
