At Agape Care Group, we extend an invitation to experienced and compassionate Social Workers to join our team dedicated to providing esteemed palliative and hospice care to patients across the Southeast. Embodying our core values of dignity and deep-seated care, our Social Workers play a vital role in enhancing our patients' quality of life by addressing not only clinical needs but also providing emotional and social support to patients and their families in their most profound moments. Perform comprehensive assessments of patient’s social, emotional, and environmental needs, formulating individualized plans to address identified needs. Act as a liaison between patients, families, and various healthcare professionals to ensure effective communication and management of the patient’s care. Provide crisis intervention and support to patients and families dealing with terminal illness and end-of-life challenges. Advocate for patient’s rights and needs within the community and healthcare systems. Facilitate access to community resources and services, including financial assistance, legal aid, and housing. Develop and lead group counseling sessions and support groups for patients and family members. Document all patient interactions, assessments, and plans in a timely and thorough manner. Strong interpersonal and active listening skills Ability to manage time effectively and handle both emotional and physical stress High level of compassion and empathy Knowledge of healthcare systems and community resources Ability to maintain professional boundaries and adhere to ethical guidelines Medical Insurance Dental Insurance Vision Insurance 401k Licensed Clinical Social Worker (LCSW) or eligibility to obtain licensure. Certifications in palliative and end-of-life care training preferred but not required.
