Job Description:

The Medication History (MedHx) Technician plays an integral role in helping to prevent patient medication errors caused by inaccurate or incomplete information in the Emergency Department. Under the supervision of a pharmacist, and according to departmental policies, procedures and guidelines, the Medication History technician will assist pharmacists in obtaining an accurate medication history by interviewing patients and consulting with their outpatient pharmacies.

  1. Obtains medication histories on all assigned patients during scheduled shifts

a. Performs patient and/or caregiver interviews to obtain dose and frequency of current medications including prescriptions, over-the-counter medications and herbal supplements

b. Calls outpatient pharmacies or other healthcare facilities to confirm medication regimens or investigate discrepancies when necessary

c. Communicates medication related issues to all appropriate heath care professionals

d. Documents medication history into the EHR per policies and procedures

  1. Cross trained in designated number of specialized areas as identified by Pharmacy specific addendum.

a. Demonstrates the ability to competently work in different (omit) general and specialized areas of pharmacy practice.

b. Provides guidance and professional knowledge pharmacy technicians and colleagues in the general and specialized areas.

c. Displays flexibility in accepting re-assignments based upon departmental needs.

d. Stays abreast of new and changing information for areas of proven competency.

  1. Fills and delivers medications under the supervision of a licensed pharmacist/licensed independent practitioner.

a Medications are prepared accurately for dispensing.

b Policies and procedures are followed consistently.

c Pneumatic tube is used appropriately for transport of meds to patients/nursing units.

d Patient identification/location is checked before delivery.

e Medications are delivered to patient/nursing units accurately.

f. Answer phones and respond to questions when appropriate

  1. Ensures adequate supplies are available for continuous patient care.

a Medications are prepared accurately for future use.

b Unused medications are returned /credited in a timely manner with accuracy using forms and/or computer.

c Medications/supplies are ordered from pharmacy storeroom/wholesaler as needed.

d Dispensing areas are left tidy & replenished after assignment is completed.

e. Out-of-date audits are performed monthly with accuracy if assigned.

g. Proper drug storage and security is maintained

  1. Participates in the training of new employees.

a Provides appropriate hands on training of new employee or student.

b Reviews and initials appropriate section of pharmacy technician checklist with trainee.

c Provides constructive feedback to trainee and degree of proficiency to supervisor.

  1. Uses appropriate computers for entry of patient information and label generation.

a Demonstrates ability to load printers/fax machine with paper/ labels/ ribbons.

b Computer use for personal use is limited to breaks/lunches.

c Patient confidentiality is ensured when using computers and processing information.

d Demonstrates working knowledge of software programs for order processing.

  1. Ensures quality assurance is maintained according to departmental and legal requirements.

a Temperature is checked on refrigerators/freezers with appropriate documentation completed. Problems with incorrect temperatures are followed-up appropriately.

b Information required for packaging/compounding is appropriately documented.

c Quality Reports are completed when appropriate in QR Tracking System.

d Documentation logs are completed, signed and kept in accordance with requirements.

  1. Operates automated dispensing devices/repackaging machine/robots/pumps.

a Medications identified accurately for restocking, repackaging, or replenishing.

b Stock is pulled/ packaged correctly for pharmacist to check.

c Refill process is documented correctly and accurately as required by departmental and regulatory guidelines.

d Clean/Maintain equipment and work areas (sterile environments are maintained according to policy and procedure

Position Compensation Range: $15.37 - $23.82 Hourly


  • Education: High School Graduate or Equivalent required
  • Experience: 6 months of experience required
  • License/Certification: Commonwealth of Virginia Pharmacy Technician License required. PTCB or NHA certification required

Job requires standing for prolonged periods. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs.
